In order to accept your aid, there are several forms that must be completed. This page houses all forms, many of which you may have already submitted or do not need to submit. If you want to know what you personally need to complete, please refer either to our email communication or the student aid portal.

Verification Documents
- Verification Worksheet
- Statement of Identity and Educational Purpose (Read the directions carefully on this document. If you are unable to present this in person with valid ID, you may get the document notarized and then mail us the original)
- Statement of Non-Filing for Dependent Students
- Statement of Non-Filing for Independent Students

Appeal Documents
You have indicated on your FAFSA that you may have unusual circumstances that may require an appeal. If such a situation applies to you, the student should initiate the appeal process by logging into their student portal and clicking the "My Financial Aid" link.
The parent or student will be emailed additional information about the appeal process, and a list of required documents based on the nature of their appeal. Once all of the requested documents have been received, the appeal will be considered and the results of the professional judgement will be communicated to the student or family within 30 business days.

SAP Documents
Direct Loan Entrance Counseling and the Master Promissory Note
Every student new to Husson University must complete Direct Loan Entrance Counseling and a Master Promissory Note (MPN) regardless if they have borrowed loans before at another school. To complete this process on your own, please follow the instructions below:
- Go to https://studentaid.gov, and sign in using your FSA ID.
- Click “Loans and Grants"
- Under “Get a Loan” select “ Loan Entrance Counseling”
- Select the correct type of Entrance Counseling.
- Select Husson University as the school you are completing counseling for
- Click “Notify this School”
- Click “I am completing entrance counseling to receive loans as an undergraduate student”
- Once you complete the Entrance Counseling, PRINT the confirmation page for your records (you do not need to send it to us).
- Under "Get a Loan" select “Master Promissory Note (MPN)"
- Click on the correct Master Promissory Note (MPN).
- Fill in all Personal Information and School Information as requested
- PAY CLOSE ATTENTION TO THE DIRECTIONS IN THE GRAY BOX AT THE TOP OF THE PAGE when completing the references section of your MPN. Failure to provide valid references will result in a rejection of your MPN, and a delay in your loan funds
- You must read each section of the terms and conditions to continue
- We will receive an electronic notification within 48 hours.
